Taking out Multiple Sefarim on Special Occasions – Vol III, pp. 293 - 294
Where is the source of the custom to take out two Sifre Torah, be it on Shabbat-Rosh-Hodesh, Yom Tob, or other such occasions?

In Seder Rab Amram Gaon [it says:] 1) "They take out a Sefer Torah and read from 'this day shall be [a remembrance] for you' [Shemot 12:14]... and then they take out another Sefer Torah and read the Maftir." From this, it can be inferred that they would take out the second Sefer Torah afterwards, when the Maftir arrived. This is reflected in Rambam's order: 2) "on every day that two or three Sefarim are read, if they took one out after the other, then when [the Hazan] brings it back he says Kaddish and takes out the next"; and see the Kesef Mishneh: "[Rambam's intention was] that if they took them out simultaneously, as is our custom today, no Kaddish is necessary." However, in Mahzor Vitry the custom to take out two Sifre Torah simultaneously was already established: 3) "at Musaf, they would take out two Torot"; and the Rosh [wrote] 4) that this cannot be found in the Talmud, but Rab Amram Gaon had it [nevertheless]. He also wrote that taking out a second Sefer derives from the Geonim: on Pesah, Shabungot, the first and last days of Sukkot, Rosh Hodesh, and Kippur, they would take out two Sefarim, and read the sacrifices from Parashat Pinehas [from the second one]. (Cf. Ran, ibid. Bene Ha-Ir, s.v. "umide'amrinan.") The Rosh [text says Mordekhai] wrote that "this matter was neither taught in the Mishnah nor the Gemara, but was established by the Saboraim or Geonim after the completion of the Talmud. It has a slight basis in [God's legendary conversation with Abraham]: 5) 'I have already established an order of sacrifices for them.'" The Hagahot Maimoniyot agreed [with this account], writing in the name of Tosafot that: 6)

בסדור העמרמי 1) ומוציאין ס"ת וקורין מן והיה היום הזה לכם וכו' ומוציאין ס"ת אחר וקורא מפטיר. מכאן משמע שמוציאין ס"ת שני אח"כ כשבא להפטיר. וכן נראה שכן סדר הרמב"ם 2) וז"ל ובכל יום שמוציאין שני ספרים או שלשה אם הוציאו זה אחר זה כשמחזיר הראשון אומר קדיש ומוציא השני וכו'. ועי' בכ"מ שם שאם הוציאם בבת אחת כמנהגנו היום אין צריך קדיש וכו'. אבל במחז"ו כבר נתיסד המנהג להוציא שני ס"ת בבת אחת 3), וז"ל ובמוסף מוציאין שתי תורות כו'. וכ' הרא"ש 4) שלא נמצא בתלמוד להוציא שני ס"ת אלא שרע"ג כתב כן. וכ' עוד דהוצאת ס"ת אחרת הוא עפ"י מנהג הגאונים דבפסח ועצרת ובימים ראשונים ואחרונים של חג ובר"ח ויוה"כ מוציאין ב' ספרים, וקורין בקרבנות היום בפרשת פנחס. וגם הר"ן שם פ' בני העיר ד"ה ומדאמרינן, והמרדכי כ' שדבר זה אינו שנוי לא במשנה ולא בגמ', ותקנוהו רבנן סבוראי או הגאונים אחר שנשלם הש"ס. וקצת סמך יש לדבר במ"ש 5) כבר תקנתי להם סדר קרבנות. וכ"כ בהגה"מ 6)ו

The custom was not taught in a Misnhah, Beraita, or Tannaitic statement... but is rather a received tradition. It does appear in the Seder Rab Amram, and it needs further study to determine whether it appears in the minor tractate of Soferim. It has a slight basis in the earlier Talmudic statement "I have already established an order of sacrifices for them," and Rabbi Chaim Benveniste wrote (ibid.) in the name of the Rosh that the Yerushalmi somewhat reflects our custom: "Ribbi Yits'hak Nafha said, 'when Shabbat falls on Hol HaMonged – be it on Pesah or Sukkot – we read from [Shemot 33:12] and then the Maftir reads from the Musaf sacrifices of Pesah in the Parashat HaMongadot in Leviticus [22]."

We should really be surprised at the Rishonim who said that the custom is "neither taught in the Mishnah nor Beraita..." and "it has a slight basis..."! Behold, in Megillah [it says] 7) "when Rosh Hodesh Tebet falls on Shabbat, we take out three Torahs and read from them – one for the weekly Parashah, one for Rosh Hodesh, and one for Hanukkah." From this it follows that on any day when there is a sacrifice [reading], we read it in addition to the weekly Parashah! Indeed, this is what Rabbi Joseph Caro wrote in the Kesef Mishneh; 8) and it is also taught in Yoma 9) and in the Gemara to Sotah 10) From this, they learned that on any day in which there is Musaf, we take out a second Sefer Torah and read the sacrifice of the day. Once the matter was mentioned once [i.e., for Hanukkah], they did not need to bring it up every single time [other holidays are discussed].
