The reason our Sages ordained that we begin the Kedusha with 'Keter'.

It says in Pirke Hekhalot that "Your servants will coronate You with crowns and sing you a New Song; and the praise they give to God is called 'crown' [Keter] because they are bringing grace to His head." You'll see that 'Keter' in alphanumerics has the equivalent value as 'God is King, God was King, God shall be King for all eternity' ["keter" = 20 + 400 + 200 = 620, and "God is King, etc..." also adds up to 620 if you use the Tetragrammaton for God's name].

Our custom is that in the Shaharit service of all prayers, the Kedusha begins with 'we will sanctify You' [Nakdishakh] (this is the view of Rab Sangadia Gaon in his Siddur), but in the Musaf service, even for a weekday Rosh Hodesh, we start with 'Your crown' [Keter]. However, Rab Ngamram Gaon wrote in his siddur that all the Ngamidot start with Keter, but the text of Keter there for Musaf is completely different from our version. By contrast, the custom in Ashkenaz is to say 'we will sanctify Your name' [Nekadesh et Shimekha] at Shaharit, and 'we will venerate and sanctify You' [Nangaritsekha Venakdishekha] at Musaf.

The Kedusha of Musaf on Shabbat and Yom Tob is called 'Kedusha Rabba' by the Posekim, meaning 'The Great Kedusha' (perhaps because of its length with respect to the Kedusha of Shaharit). In the Zohar, Parashat Vayak'hel (p. 92) it is called 'The Supreme Kedusha,' and I found another place in the Zohar, Parashat Pinehas (p. 242b and the Tikkune Zohar, Tikkun 21) that calls this Kedusha simply 'Keter': "The Tanaim and Amoraim held that in the Musaf service of Shabbat and Yom Tob, we say Keter..." look there.

Why our Rabbis ordained that we say the verse "Shemang Yisrael" [Deuteronomy 6:4] in the Keter; and why the custom of the Land of Israel and in Syria, Togarma, and Egypt is that the public says it first and the Hazan repeats it, while in London and Amsterdam the Hazan says it at the same time as the public.

Regarding reason that we say the verse Shemang Yisrael in the Kedushah of Keter, Abudarham wrote (p. 35, column 4):

טעם שאומרים פסוק שמע ישראל בקדושת הכתר, כ' אבודרהם (דל"ה ע"ד)ו
|
It is because [Justinain I] issued a decree against the Jews that they could not recite the Shemang [in the usual long form at Shaharit and Ngarbit]. The Rabbis ordained that the Hazan say just Deuteronomy 4:6 as an allusion [to the long Shemang] at every Shaharit service during the Ngamidah on both weekdays and Shabbat, and for this reason they also repeated it or answered after the Hazan in order to be absolved of the requirement to recite the Shemang. (Author's note: through this we can understand the reason behind the practice in the Land of Israel and in Syria, Togarma, and Egypt to repeat [this line], which is that they didn't want to change the ancient custom even though we are free to recite the Shemang now without any legal restraint. On the other hand, the explanation of the custom of London and Amsterdam is that now that we can recite the Shemang, there is no longer any need to repeat it.) When [Justinian's] decree was rescinded, the Rabbis ordained to cancel the Shemang from [the Kedusha of] all the services, but established that we say it in the Musaf service since the long Shemang does not appear in Musaf, and also as a way of publicizing the miracle forever by thanking God for the rescinding of the decree – thus wrote the Geonim.

משום שגזרו שמד על ישראל שלא לקרות ק"ש תקנו שיהיה ש"ץ אומרו בהבלעה בכל תפלת שחרית בעמידה בין בחול בין בשבת, ומטעם זה כופלין אותו או עונין אחריו כדי לצאת י"ח ק"ש (אה"מ בזה אנו מבינין בטעם א"י וסת"מ שכופלין אותו משום שלא רצו לשנות מהמנהג הקדמון אף שעתה יכולים אנו לקרות ק"ש ואין מוחה בידינו, אבל בטעם לונדון ואמש' מכיון שאנו קוראין ק"ש שוב אין צורך לכופלו) וכשנתבטל השמד בטלו לומר כן בכל התפלות וקבעו לאומרה בתפלת מוסף לפי שאין בתפלת מוסף ק"ש, והשני כדי שיתפרסם הנס לדורות ולהודות לה' על ביטול השמד, כן כתבו הגאונים עכ"ל

How nice would it have been if the Rabbis had also ordained that we respond "blessed be His holy name forever and ever!" as is our custom in the long Shemang [cf. Mishna Yoma 3:8, 4:1-2, and 6:2]

In the book Nahal Kedumim on the Torah, at the end of Parashat Behar, the gifted Hida assumed that this is a Scriptural verse because the text says 'Dikhtib' [other texts have the synonym 'Lemor,' which brings up the same issue]. This word is used by all the sages of the Talmud, as well as the Rishonim and Aharonim, as a way to introduce a verse (or they use 'Shene'emar' as on the first folio of Berakhot). But the truth is that it is not a verse but rather the words of the liturgist who established the Kedusha of Keter (cf. Ben Ish Hai's Tikkun Tefilla, p. 366).
